College of Health Solutions application packet
Application packets need to be submitted to the graduate support coordinator before June 1 to begin taking graduate coursework for the fall semester and Oct. 1 for the spring semester. If a student is determined to be a strong candidate for the 4+1 program they will be given permission to take graduate-level courses. The student will then formally apply to the graduate program for the semester after they complete the bachelor’s degree program.
All applications must be submitted to the contact electronically. Each electronic application packet should be sent in one email and include the documents listed below.
Biomedical informatics, BS / Biomedical informatics, MS
- Copy of unofficial ASU transcript (3.0 minimum)
- Statement of purpose (1-2 pages)
- Three (3) letters of recommendation from academic sources are preferred.

Clinical exercise science, BS / Clinical exercise physiology, MS
- Copy of unofficial ASU transcript (3.0 minimum)
- Personal statement (1-2 pages)
- Three (3) letters of recommendation
- Two of the letters must be from faculty members within the College of Health Solutions. The third letter needs to be from Clinical Professor Donna Cataldo, donna.cataldo@asu.edu.

Clinical exercise science, BS / Physical activity and health, MS
- Copy of unofficial ASU transcript (3.0 minimum)
- Personal statement (1-2 pages)
- Within this statement, identify the faculty member in the major that will serve as the 4+1 advisor. Be sure to request this of the faculty member before identifying them.
- Letters of recommendation
- Three (3) letters of recommendation will be requested when officially applying. Two of the letters must be from faculty members within the College of Health Solutions.

Dietetics, BS / Nutritional science, MS
- Paper Application
- Copy of unofficial ASU transcript (3.0 minimum GPA)
- Personal statement: provide a 1-2 page personal statement that:
- Describes the significant professional responsibilities you have held.
- States your professional goals and reasons for desiring to enroll in this ASU MS program.
- Describes your strengths that will help you succeed in the program and in reaching your professional goals.
- Indicates your personal research interests, including any previous research experience you may have acquired.
- Three (3) letters of recommendation:
- Letters of recommendation should be written by those who can speak to your aptitude for research and master’s level coursework.
- One letter must be from an ASU nutrition faculty member.

Speech and hearing science, BS / Auditory and language neuroscience, MS
- Copy of unofficial ASU transcript (3.0 minimum)
- Three (3) letters of recommendation.
- Letters of recommendation should be written by instructors, research mentors and/or clinical supervisors who can speak to your aptitude for research and master’s level coursework.
- Personal statement
- Please include in your statement three potential research mentors with whom you would like to work, and why. The program will work to pair students with mentors based on research and training interests.

Thunderbird application packet
Refer to Thunderbird’s Master of Global Management degree page for more information and contact admissions.tbird@asu.edu before starting the admissions process. Note the application deadlines for the 4+1 accelerated programs with Thunderbird, as they are different from the deadlines at the College of Health Solutions.
You are eligible to apply if you:
1. Are an on-campus student able to take courses at the Downtown Phoenix campus in your senior year
2. Have at least a 3.0 ASU Cumulative GPA. No GMAT or GRE required.
3. Have completed at least 75 credit hours at the time of application (Junior/Seniors are eligible)
4. Have room in your schedule during your senior year for 12 credits of Master of Global Management coursework
Applicants are required to:
1. Have an updated resume documenting professional and academic history
