Call for Proposals

Presentations should be based on research evidence, have clear learning outcomes and relate to this year’s conference theme and goals.


Integrating medical, psychological and social systems to treat patients is key to improving overall population health, and using a systematic, coordinated approach is critical to achieving the Quadruple Aim of health care. 

We seek proposals that take a variety of approaches to population management in integrated care, such as:

  • Improving the prevention and management of chronic medical, lifestyle and behavioral conditions. 
  • Addressing social determinants of health, health disparities and community-based approaches as they relate to population health management and integrated care.
  • Solving health problems with team-based approaches that include inter-professional collaboration.
  • Improving the efficiency and fiscal sustainability of integrated care programs with the use of health technologies like patient registries, predictive modeling, machine learning or financial systems.  

Submit your proposal online by Sunday, Feb. 23, 2020. Submissions will be evaluated by the conference committee for relevance and audience appeal. Proposals must be complete in order to be considered. Accepted submissions will be notified by mid-March.
